Nursing Foundations

Purpose

To provide an opportunity for the student to create a teaching plan that will meet the learning needs of a client. You will be assessed on how you apply the following General Learning Outcomes:

● Integrate the nursing metaparadigm, nursing theory, and related knowledge into professional nursing care

● Apply principles of teaching and learning to basic nursing care and to client teaching

● Demonstrate reflective practice for personal and professional growth and continued competence

● Examine research findings that support evidence of informed practice in the delivery of basic nursing care

Instructions

1. Choose a scenario from the list at the end of this document (p.4).

2. Read the scenario and complete the following:

Assessment Data & Learning Needs:

Scenarios

Mrs. Jackson

You are working at a continuing care centre and your client Mrs. Jackson is 75 years old, has a diagnosis of COPD and arthritis. She is widowed and has a daughter that lives in the city and sees her once a week. Mrs. Jackson is unsteady on her feet and recently had a fall. An assessment was done by physio and it was determined that she would need a walker. Mrs. Jackson states she does not like to use her walker as she thinks it is a sign of weakness and dependency. Mrs. Jackson is on oxygen and Advair (inhaler) for her COPD. She is also hard of hearing and wears hearing aids.

Mr. Yashimoto

You are working at a continuing care centre and your client Mr. Yashimoto is 70 years old, has a history of atrial fibrillation and congestive heart failure (CHF). He is widowed, speaks very little English and has two children who live close by (both speak English). Mr. Yashimoto is taking an anti-arrhythmic and blood thinner. Mr. Yashimoto was recently started on Lasix (diuretic). Mr. Yashimoto has noticed that he is going to the bathroom more frequently. Mr. Yashimoto uses herbal products and eats little of the food provided to him, as he does not like it.

Mrs. Yakimets

You are working at a continuing care centre and your client Mrs. Yakimets is 65 years old has a history of hypertension. Mrs. Yakimets was recently admitted tonthe continuing care centre. Mrs. Yakimets had a hemorrhagic stroke, which has left her with right sided weakness and slurred speech. She is overweight, hard of hearing, and wears a hearing aid. She is married, her husband lives in their family home and her daughter lives in British Columbia.

Mr. Akal

You are working at a continuing care centre and your client is Mr. Akal, 80 years old and is blind due to glaucoma. He has a history of Type 2 diabetes and takes Metformin 1000 mg PO BID and a long-acting basal insulin. His most recent fasting blood sugars have been 11.9 mmol/L and 13.0 mmol/L. His physician has now ordered basal bolus insulin therapy to start with a target range of 5-10 mmol/l. Mr. Akal has noticed increased urination which has resulted in episodes of urinary incontinence. Mr. Akal is widowed and has a daughter that visits frequently.
